Both of these medical plan options offer the flexibility to see any doctor you choose in the Blue Cross Blue Shield of Michigan provider network. The PPO 3600 + HSA includes a Health Savings Account (HSA) option, so you can set money aside to pay for current and future health care expenses.

All pre-Medicare retiree medical plan options require a monthly contribution. Ford continues to contribute a fixed amount toward the cost of retiree medical benefits, based on your level of coverage. Ford's contribution is already included in the monthly contribution amount you see in the preceding medical plan comparison chart and on myfordbenefits.com. Focus on the Health Savings Account

If you enroll in the PPO 3600 + HSA, which has a Health Savings Account (HSA) option, here are some important things to know:

  • Funding. You can fund the HSA with your own money to pay for current and future qualified health care expenses.
  • Making contributions. You can contribute up to $3,600 if you cover yourself only and up to $7,200 if you cover yourself and other dependents in 2021. When you make contributions to your HSA, they're made on an after-tax basis directly to HealthEquity, the administrator of the HSA. Make contributions by accessing Make Contribution, under Quick Links on the HealthEquity website at my.healthequity.com. Note: Pension deductions are not available.
  • Opening an HSA. You can open an HSA when you enroll for your medical plan. You'll accept the terms and conditions during the enrollment process and then enroll directly with HealthEquity. Keep in mind that if you already have an HSA, you don't have to open a new one.
  • Change your contributions any time. You can change your HSA contribution amount any time throughout the year.
  • Direct access. You can access your Health Savings Account directly through HealthEquity at my.healthequity.com.
  • Invest your HSA funds. You can invest the funds in your HSA to help it grow. When your account balance reaches $1,000, you can choose to invest in a selection of mutual funds. To use invested funds for current eligible expenses, just transfer some or all of the money back into your HSA. The advantages of investing in mutual funds is that they:
    • Are managed professionally
    • Offer a diverse portfolio
    • Are affordable
    • Have high liquidity
    • Provide tax savings

Coverage when you are Medicare eligible

When you become eligible for Medicare, Ford provides access to Via Benefits. Via Benefits provides Medicare-eligible participants with personal assistance and the opportunity to evaluate, select and enroll in Medicare plans from leading health insurers. The Via Benefits marketplace offers Medicare Supplement Insurance (Medigap), Medicare Advantage and Medicare Part D prescription drug plans, as well as dental plans. One year before you reach age 65, you'll receive a postcard from Via Benefits with details about the program and things to consider as you approach Medicare eligibility. Enrolling in Medicare

You become eligible for Medicare when you reach age 65. For your convenience, you can enroll in Medicare and apply for Social Security benefits at the same time — as early as three months before you reach age 65. If you are under age 65 or disabled and are already receiving Social Security benefits, you'll be automatically enrolled in Medicare Part A as of the first day of the month in which you reach age 65. If you wait to enroll in Medicare until you're already age 65, the start of your Medicare coverage may be delayed.

Split coverage rule

Don't forget that if you are pre-Medicare eligible and your spouse is Medicare-eligible and receiving Health Reimbursement Account (HRA) contributions, you must enroll in a medical or dental plan for 2021 to ensure your spouse continues to receive contributions to their HRA. If you don't enroll, you will lose medical or dental coverage, and your spouse's HRA contributions will end for 2021.

Michigan auto reform reminder

Michigan Auto Reform Reminder

While you're making your benefits decisions, you may want to review your auto insurance based on the recent auto reform legislation. The state of Michigan made changes to no-fault motor vehicle insurance that went into effect July 2, 2020. The new law allows drivers to opt out of Personal Injury Protection (PIP) or purchase a reduced level of PIP or unlimited PIP coverage. Drivers can only opt out of PIP coverage if their health insurance covers auto injuries and meets Qualified Health Coverage (QHC) status criteria as defined by the auto insurance reform law. QHC is defined as Medicare or an employer-provided health plan that does not exclude auto injuries and has an individual deductible of $6,000 or less per individual. To find out if your plan is QHC, go to myfordbenefits.com and click the Michigan Auto Reform tile on the home page.

While the law may offer an option to lower auto insurance premiums, we recommend you carefully review the benefits provided under PIP before deciding whether to limit or opt out of PIP coverage on your auto insurance policy. Contact your insurance agent for PIP coverage details. Michigan auto insurers can request proof of Ford QHC if an individual is lowering or opting out of medical PIP.

About maintenance medications

If you take maintenance medication to treat an ongoing condition, such as high blood pressure, heart disease, asthma or diabetes, you have a few options for filling your prescription.

You can only fill a 30-day maintenance medication prescription at a retail pharmacy up to three times. Prior to the fourth fill, you will be required to ask your doctor for a 90-day prescription using one of the two options below:

  • Use the Express Scripts Home Delivery Program. Ask your doctor to submit a 90-day prescription to Express Scripts Home Delivery and get your medication delivered to your home at a reduced price.
  • Ask your doctor to submit the prescription to your local participating Walgreens. With Walgreens Smart90 program, you can pick up your 90-day prescription at Walgreens at the same reduced price as the Express Scripts Home Delivery Program.

Other benefits

Ford also offers other valuable benefits, including dental, vision, group legal, optional life insurance and voluntary benefits.

Dental

Ford offers two dental plan options to choose from:

  • Dental HMO coverage through DeltaCare® USA, which is an HMO-type nationwide dental plan option. You choose from primary care dentists who are part of the DeltaCare USA DHMO network and pay pre-set copayments for services; there are no annual maximums or deductibles. If you enroll in the DeltaCare USA DHMO, you'll have coverage only when you use in-network providers. There is no coverage if you use an out-of-network provider in the DHMO.
  • MetLife Traditional Plan, which has a network of preferred dentists called the Preferred Dentist Program (PDP). You can use any dentist, but if you choose a network dentist, your costs are generally less. The amounts paid by the plan are based on amounts that PDP dentists have agreed to accept. Out-of-network dentists could charge you more, even if they're willing to bill your MetLife insurance.

Life and Accidental Death and Dismemberment Insurance

Basic Life Insurance and Accidental Death and Dismemberment (AD&D) Insurance are provided to eligible retirees up to a maximum of $25,000. For eligible Disability Retirees, your coverage reduces to a maximum of $25,000 at age 55. AD&D Insurance ends at age 65.

If you are an eligible retiree and were enrolled in Optional Life Insurance, Dependent Life Insurance (Spouse or Child) or Optional AD&D Insurance immediately before retirement, you are automatically enrolled in the same level of coverage you had as an active employee (some limits may apply). As a retiree, you cannot increase coverage, but you can decrease or cancel your coverage any time. Retiree rates apply.

Note: For Optional Life Insurance (retiree coverage) and Dependent Life Insurance (spouse coverage), your contribution rate will be based on your age as of December 31 of the plan year.

Critical Illness Insurance

Critical Illness Insurance supplements your medical plan by providing a lump-sum payment you can use for whatever you need, such as paying for deductibles, living expenses, mortgage or rent, caregivers, etc. Note: You can only enroll in Critical Illness Insurance during Annual Enrollment. You'll be able to enroll through the voluntary benefits website, which you can access from the confirmation page after you enroll on myfordbenefits.com.

Hospital Indemnity Insurance

Hospital Indemnity Insurance can complement your medical coverage to help ease the financial impact if you're hospitalized. It provides a lump-sum payment that can be used for a hospital admission, accident-related inpatient rehabilitation and hospital stays — or any other expenses you may have. Costs can include plan deductibles, copays and co-insurance for doctor and emergency room care, testing and supplies, and extra costs for out-of-network care, which may not be covered. Note: You can only enroll in Hospital Indemnity Insurance during Annual Enrollment. You'll be able to enroll through the voluntary benefits website, which you can access from the confirmation page after you enroll on myfordbenefits.com.

Auto Insurance

Through the voluntary benefits program, you can get side-by-side quotes from some of the nation's leading auto insurance companies. Special employee discounts are available as well as the opportunity to pay your premiums through pension deduction. Coverage is competitively priced and reviewed regularly to make sure the rates are among the best in the marketplace.

Homeowners Insurance

Through the voluntary benefits program, you can get discounts from two of the top-rated carriers in the nation on homeowners and renters insurance.

Identity Theft Protection

Safeguard your family from the threat of identity theft with leading-edge technology that detects fraud at the source to help catch misuse sooner and minimize damages. The plan provides tri-bureau monitoring with unlimited credit scores, annual credit reports with dark web monitoring and data breach notifications. It also provides up to $1 million in identify theft insurance protection.

Pet Insurance

Protect yourself from the high costs of veterinary care. You can choose the plan that works best for you with benefits paying 90%, 70% or 50% back on veterinary bills. Use any veterinarian, anywhere. The plan provides the same price for pets of all ages.
